The Note node type can be added to any survey using Survey Designer. The Note allows you to embed notes for the survey author into the survey. Typically that might be for additional survey intelligence that needs to be finished off later, for example appropriate skip logic that needs to be added, masking, additional sections that are required etc. You could use script nodes or folders, but the Note node is dedicated specifically for this purpose. The notes are for design information; they are not visible to respondents.
Notes are not language dependent, so there is only one field even if several languages are active in the survey. Note nodes can only be added and edited from Survey Designer, and while they are visible in the Professional Authoring tree they cannot be added or edited there.
Click on the node in the Survey pane to open its details page.

To change the name of the node, click on the Edit icon and edit as required. Type your notes into the text field.
