All the question types that you can add to a survey are stored in and accessible from the Add Item overlay.
Click once on a question type to add it to the survey.
You can click the down-arrow that is presented beside some of the question types to open a list of the Appearance options for that type. You can then create that type directly instead of having to change the Appearance option later.
Some question types are only available when specific modes are selected (go to Survey Settings for more information).
